完全胸腔镜下解剖性肺段切除术治疗早期肺癌的体会马少华;梁海龙;闫天生;王可毅;王京第;宋金涛;王通;贺未;白洁;金亮【摘要】BackgroundandobjectiveSegmentectomycanretainsmorehealthylungtissuethanlobectomy,butitremainscontroversialinoncologyforearlystagelungcancer.Theaimofthisstudyistodiscusstheproblemsofvideo-assistedthoracicsurgery(VATS)segmentectomyinearlystagelungcancer,byanalyzingtheclinicalandpatho-logicaldataof35casesandreviewingtheliterature.MethodsTherewere35patientswhoreceivedsegmentectomybycompletevideo-assistedthoracicsurgery,fromMay2013toJuly2017,insingleoperationgroupintheThirdHospitalofPekingUniversity.Weanalyzedthepatient'sclinicalandpathologicaldata,intraoperativeandpostoperativecom-plications,lymphnodenumberandmetastasisitssituation,andcomparedpostoperativepathologyandpreoperativecomputedtomography(CT)imagingtype.In35casesofsegmentectomy,therewere11malesand24females,withanaverageageof57.7yearsold.Thelesionslocatedintherightupperlobewere8cases,intherightlowerlobewere8cases,intheleftupperlobewere13cases,intheleftlowerlobewere6cases.ThemeanmaximumdiameterofCTimagingwas12.7mm,andthelargestdiameterofhilarandmediastinallymphnodeswaslessthan10mm.23ofthemweregroundglasspredominatingand12weresolidcomponentspredominating.ResultsAll35casesweresuccessfullycompletedVATSanatomicalsegmentectomy.Theaverageoperationtimewas153minutes,theamountofbleedingwas51mL.Therewere10casesofairleakageafteroperation,allofwhichwerenotmorethan3days.Therewascontralat-eralatelectasisin1case,chylothoraxin1case.Theaveragelengthofhospitalizationwas6.1days.Therewasnoothercomplicationsoutpatientrelatedtosurgery,in30daysafterdischarge.Thepathologicalchangeswereasfollow,2casesofmetastatictumor,8casesofbenignlungdiseaseand25casesofprimarylungcancer.Inthe25casesofprimarylungcancer,therewere14casesofinvasivelungadenocarcinoma(7casesweregroundglassopacity(GGO)predominatinginCTimaging),4casesofmicro-invasiveadenocarcinoma(3caseswereGGOpredominatinginCTimaging),6casesofadenocarcinomainsitu(allw
